It was in the setback pocket.

Material : Stiff paper cover with soft paper on the inside.
Location of loss : Left in the seat back pocket. The flight was from Fort Lauderdale to Phoenix. I went to the airport in Phoenix so I think it is in Fort Lauderdale.
