Description of lost item(s) : I

 lost my document folder containing the following: x1 Work Permit (UCI – 11*****023, A*** B***) x1 SIN number print (9**-***-505 for A*** B***) x1 Offer Letter from Infosys Limited (dated 28 Feb 2026 for employee id: 11*****56, A*** B***).

All the 3 documents were enclosed inside a clear plastic folder of A4 size. The incident happened on 8th July 2026, evening.

The location was Notre Dame De Montreal, square. The folder kept on the bench facing the bank (right next to the statue).

The item was left at 5:44 pm unattended on the bench. Once I returned back at 6:35 pm, it was missing.

Location of loss : Notre Dame De Montreal’ central square. Folder was last kept on one of the benches facing the bank, right next to the central statue. It was a clear plastic folder with 3 documents, out of which 2 were issued by Canadian government bearing the name A*** B*** in all 3. City : Montreal
